EH&S published an updated UW Confined Space Program Manual that reflects revisions to the Washington State Department of Labor & Industries confined space regulations (2018 update) and aligns with standards established by federal Occupational Safety and Health Administration (OSHA).

Students enrolled in the Health Sciences Immunization Program can submit the COVID-19 Vaccination Medical Exemption Form (Word) to initiate University review. The form must be completed by a licensed healthcare provider and submitted according to the instructions on the form.
The Permit-Required Confined Space (PRCS) Entry Form is required for entry into any permit-required confined space. The PRCS Entry Form may be used for either an alternative methods entry or a permit entry depending on the hazards present in the space and the ability to eliminate or control the hazards.
