The Department of Labor and Industries, Division of Occupational Safety and Health (DOSH) granted a variance to the University of Washington regarding employee­-elected safety and health committee members. 

Download the UW Diving Medical History Form and return the completed form to the UW Employee Health Center.

The UW Medical Evaluation for SCUBA Diving form must be completed by a physician: Send completed form and copies of original test results to UW Employee Health Center (EHC) via email at emphlth@uw.edu or fax at 206-221-5110. 

The U-Wide Health and Safety Committee (U-Wide) will convene per the University of Washington (UW) Executive Order 55. Members include elected, appointed, and Ex Officio employees. Meetings are conducted utilizing Robert’s Rules of Order. The U-Wide reports to and advises the UW Environmental Health & Safety Department (EH&S) senior director.

1. Smoking tent in Physical Plant parking lot
2. Smoking tent south side of UW1
3. Smoking tent west side Campus Way NE across from CC1/CC2
4. Smoking tent in UWBB upper parking lot
