Air Permit
All construction projects and air pollutant sources at the University must comply with the University’s Air Operating Permit requirements and all related policies and regulations. Environmental Health & Safety (EH&S) provides guidance, consultation, regulatory oversight, and assistance with environmental quality compliance and permits.
View the Air Operating Permit issued to the University on the Puget Sound Clean Air Agency website.
Lockout/Tagout Authorized Person(s) Periodic Review
The Lockout/Tagout Authorized Person(s) Periodic Review is used to inspect the Authorized Person’s role and responsibility established in the hazardous energy control program and confirm that the Authorized Person has sufficient knowledge of hazardous energy control procedures and their application to lockout/tagout and tagout only.
Refer to the UW Hazardous Energy Control LOTO Program Manual for more information.
LOTO Lock Removal Form
The LOTO Lock Removal Form (Word) includes directions and specific information a department or unit must document when an Authorized Person (lock owner) is unable or unavailable to remove their personal lock during a LOTO procedure.
Refer to the UW Hazardous Energy Control LOTO Program Manual for more information.
UW LOTO Procedure
For equipment that is routinely maintained and serviced, requires a full shutdown for servicing, and has good change management practices in place, the UW LOTO Procedure form can be used.
Refer to the UW Hazardous Energy Control LOTO Program Manual for more information.
Guidance for Temporarily Reclassifying Permit Required Confined Spaces
In some cases, permit-required confined spaces can be reclassified as non-permit spaces. Download the guidance document below and follow the instructions to temporarily reclassify a permit required confined space.
Visit the Confined Space Program page for more information.
Confined Space Evaluation Form
If you suspect a space meets the definition of a permit-required confined space, but you do not have a permit to enter, do not enter the space. Complete the Confined Space Evaluation form and submit it to EH&S for evaluation.
Hazardous Energy Control - Lockout/Tagout
Hazardous energy, such as electricity, chemical, radiation, pneumatic, hydraulic, mechanical and gravitational energy sources, must be isolated and dissipated before servicing or maintaining equipment. Hazardous energy controls are generally known as lockout/tagout, or LOTO, and consist of locks and tags applied to energy-isolating device(s) on equipment that contains the hazardous energy source(s).
Confined Space Program
A confined space is one configured so that a person can fully enter and work, but is not designed for continuous human occupancy and has restricted or limited means of entry or exit. The UW Confined Space Program applies to any UW unit, department, or organization that has space(s) that may potentially be hazardous when entered.
Walking-Working Surfaces Inspection Checklist
Units and departments can use the Walking-Working Surfaces Inspection Checklist to identify areas or conditions for potential slip and trip hazards, especially in office and general work areas.
Refer to the Fall Protection page for more information.
