Why did I receive a Fume Hood Condition Notice?
During routine testing of your fume hood, our surveyor identified conditions that present a potential hazard to the lab and its occupants. These conditions are typically the result of equipment failure or worker practice.
What should I do?
Below are questions asked during safety equipment inspections and explanations for each question.
The Occupational Hearing Loss Focus Sheet contains Information about hearing loss and how it may affect University personnel who may be exposed to noise.
The purpose of the Hearing Loss Prevention Self-Audit Form (Word) is for University units to audit their compliance with the Hearing Loss Prevention Program.
This form applies if the unit has personnel enrolled in the Hearing Loss Prevention Program or has identified high noise areas that require signage.
University units/departments may use the UW Float Plan as a template to develop their float plan.
