Biological Use Authorization (BUA) application (new and renewal)
Complete the Biological Use Authorization (BUA) application for new and renewal projects to obtain approval for research involving biohazards. Submit BUA applications at least four weeks prior to the IBC meeting. Electronic submissions to ehsbio@uw.edu are preferred. Submit via campus mail to EH&S at Box 357165 or by fax to 206.221.3068.
