University employees and supervisors can prevent accidents and incidents from recurring when they identify correctible root causes and take the appropriate corrective actions. This training displays how root cause analysis works and let's you practice the Five Whys Technique for incident investigation. The training also identifies how to document incident investigations in the Online Accident Reporting System (OARS).
