Certificate of Environmental Compliance

Grant Process

During the grant process, some funding agencies (especially USDOD and USEPA) require an Environmental Compliance review. This review requires the Responsible Facility Official (RFO) to sign a Certificate of Environmental Compliance. The UW RFO for environmental compliance issues is the Department of Environmental Health and Safety Director.


Obtaining Review and Signature

  1. Complete an EH&S Lab Safety Checklist (Word).

  2. For a proposed project when an EH&S review has not been completed, submit a completed Research Project Hazard Assessment form with an abstract, your completed Lab Safety Checklist, and the Certificate of Environmental Compliance form provided by the granting agency to the Institutional Biosafety Officer.

    For an existing project, assure that an EH&S review has been completed. The EH&S review should have a reference number assigned to the project. Submit a signed letter with the EH&S reference number, your completed Lab Safety Checklist, and the Certificate of Environmental Compliance form provided by the granting agency to the Institutional Biosafety Officer.

  3. Submit a signed copy of the Principal Investigator Safety Program Assurance form (this form should be in your DOD packet).
