Requesting a Non-Routine Waste Pickup

If you will generate a certain waste only once or a few times, you must complete and send us a Chemical Collection Request each time to have your waste picked up by EH&S.

Chemical Collection Request forms may be printed from our website or you can call 206.616.5835 to request that one be mailed to you.

Complete the form as shown below, with contact name and information. Indicate the name and amount of all chemicals and the percentage of each in the container, including water. Make sure that all percentages of each constituent in your waste add up to 100%.

When you have completed and signed the Chemical Collection Request Form, either mail it to EH&S Hazardous Waste Office, MS 354400 or FAX to 206.685.2915 . Normally the chemicals will be picked up by EH&S within two to four weeks.
