Note: Once entering OARS, you will have approximately 30 minutes to click
SAVE, SUBMIT, GET PDF, or any of the SELECT buttons before the connection to OARS times out.

Make sure Pop-Ups are enabled for oars.ehs.washington.edu.
You cannot complete a report if Pop-Ups are disabled. |

Enter OARS and click the Create New Incident Report link.

After filling out the form, click the GET PDF button.
Save the generated PDF Accident Form.
If you do not send this PDF form to EH&S by mail or email the report could be permanently lost.

Once the PDF Accident Form is saved and/or printed, click the SUBMIT
button.
Note: You may instead click the SAVE button if you are not finished and wish to
complete the Accident Form at a later date.

Once you have Submitted your OARS accident report, do one of the following things:
- Print your saved PDF Accident Form and mail it to:
OARS - EH&S, Box 354400
OR
- Email your saved PDF Accident Form to:
injury@u.washington.edu

If this happens, you must exit completely and re-enter OARS, losing any unsaved
form data, to re-establish a connection.
These special instructions are designed to address technical problems with the OARS
system.
