Fire Alarm Evacuation Exemption

Purpose 

This request form may be used to apply for an exemption from evacuation during activation of the building's fire alarm system. It is intended for use by staff associated with a medical procedure in various buildings at the University of Washington main campus, including ambulatory fire zones at the Medical Center, the Dental School, the Primate Center, etc., where complete evacuation is required during a fire alarm. The intent of this exemption is to reduce the possibility of compromising a patient or animal's health by stopping, or delaying completion of a surgical or other significant medical procedure.

Instructions

  1. A separate request should be completed for each location where evacuation is deemed necessary.
  2. Applications must identify a health exposure for approval; those outlining inconvenience, loss of research, data, test results and the like will not be approved.
  3. To apply for an exemption, complete the request form and either submit it via email online, or route it for approval to Darren Branum, Environmental Health and Safety, Box 354400.
  4. Approved forms will be distributed to all parties and must be included in departmental evacuation plans.  A copy must also be posted at the exempt location and at the Fire Control Center.  The University Police Department will maintain a copy for their reference.
